Instructions for the Domestic Mentor Program Student Enrollment Form

1. Review the biographies Domestic Mentors at: z.umn.edu/2017_18hhhmentorbios.

* New biographies are being added. Please check back regularly to view additional biographies.

2. Choose up to 3 people you would like to work with and why you feel a mentor like that would be useful for you. We will use your preferences & explanations to guide the matching process.

* We can not guarantee you will be matched with one of your 3 choices, but you will be matched with a mentor with similar interests/experiences.

3. Write a brief essay about your career interests and what benefits you hope to gain from this mentor program, specifically answer the following: 1.) reflect on your career interests and why you are a student at the Humphrey School; 2.) reflect on how a connection with a professional in the field can help you explore your career interests. Essay should be 250 words maximum.

4. If you cannot find any person in this list who is a good match for your interests, please address this in your essay. Clearly explain what types of knowledge and interests you are looking for.

5. The priority deadline for submitting the enrollment form is September 5, 2017. The final deadline is September 15, 2017.

The matching process will begin in September, and you will be informed of your match in late September via e-mail.

Remember: the information you provide is crucial to making the best match for you, so please be thorough in your answers.
