P2/3 Report an Absence/Remote Participation
Students in the P1 class please fill out this form instead.

Is your absence more than two days, or will you miss required course components? If so, please proceed. 

For conference-related absences, please submit your absence to the Student Conference Attendance Form. More details can be found in the Procedure for Professional Conference-Related Absences.
 
Process:
  1. OSS staff will review your request and consult with your advisor, if necessary, to ensure that you have support during your absence and upon your return.
  2. Upon approval*, OSS staff will notify instructors (and copy you) of your absence and expected return. 
  3. When you return, it is your responsibility to arrange make-up work directly with course instructors. 
  • You should provide 21 days advance notice for all foreseen absences such as religious observances, naturalization, scheduled medical procedures, etc.
  • Students who experience respiratory virus symptoms (including COVID and influenza) should follow CDC guidelines with regard to isolation and testing. . 
  • Please follow the Class Attendance Guidelines which note that "students with patterns of absences may be required to produce verification of the absences from a third party."
