EXHIBITOR AGREEMENT
$600 Exhibit Fee Includes:
-  One 6-foot table for a table-top display and 2 chairs located outside meeting room
-  Continental breakfast, refreshment breaks, and lunch for 2 representatives
-  Acknowledgement in course materials and signage
-  Observation of educational activity for 2 representatives
-  List of attendees (Opt in list)
-Note: Confirmed exhibitors are permitted to have a maximum of two representatives present at the exhibit table and on-site at the activity at any time.

On-Site Policies:
- Representative(s) must wear a company name badge at all times. Please bring a badge with you; they will not be provided on-site.
- Please plan to provide the education coordinator, Laura Walton, with your business card.
