4-H Family Tag Request Form
Use this form to request tags, taggers or pins for your family's use in 2020 from your local extension office.
NOTE:
All tags must be put in the animal's ears and recorded in 4HOnline by June 1, 2020.  
Any unused tags (tags beyond those you listed in 4HOnline) must be returned to the Extension Office by July 6, 2020.
